Day 5 pain after wisdom tooth removal is the point where a lot of people get worried because the pain seemed to ease and then flared again. That middle stretch can be normal, but the pattern matters.
A healing socket, a dry socket, and a sore jaw do not usually feel the same. The easiest way to sort it out is to match your pain to the telltale pattern.
Day 5 pain usually comes from one of three causes
Day 5 pain after an extraction often comes from normal healing irritation, dry socket, or muscle and jaw joint strain from the procedure. A lower wisdom tooth that was impacted or hard to remove can leave more bruising and stiffness, so the recovery can feel uneven.
One clear line matters here: severe pain that is getting worse on day 4 or 5, especially if it radiates to the ear or temple and pain medicine barely touches it, needs the dentist who removed the tooth. That pattern fits dry socket more than routine healing.
- Normal healing irritation: dull to moderate soreness, tenderness when chewing, gradual overall improvement even if one day is worse.
- Dry socket: sharp or throbbing pain that ramps up after initial improvement, often stronger on days 3 to 5 and hard to ignore.
- Jaw muscle or joint strain: aching when opening wide, chewing, or moving the jaw, often with stiffness more than socket pain.
If you’re worried about the removal itself you’re worrying about the wrong thing. It’s the recovery that sucks. It’s so tedious, you can only eat certain things and for the first couple of days you can only consume liquids. I was more worried about dry socket than anything else. You’ll get through it though. I’m on day 5 post surgery and I’m past liquids and currently on soft foods.

Normal healing can still hurt on day 5
Normal healing pain on day 5 is usually local soreness at the extraction area with gradual improvement from day to day. Eating, rinsing, talking a lot, or sleeping on that side can make it flare for a while without meaning anything has gone wrong.
A healing socket can also feel strange rather than just painful. Tender gum edges, a bad taste from trapped food, mild swelling, and an empty-looking hole can all happen during normal recovery.
A slow recovery is more common after a difficult surgical extraction, after several teeth were removed, or when the tooth was deep in bone. Some people still need pain relief beyond day 5 and are healing normally.
I’m 10 days post op and I still some pain on my left side. No dry socket, everything is healing nicely. I just take 200mg once a day of ibuprofen and that seems to help. Wisdom teeth removal is a very traumatic procedure. There are so many nerves and bones in your mouth that were touched or removed. I’m just trying to tell myself that it will heal one day.

Dry socket pain has a more specific pattern
Dry socket pain usually starts after the first day or two, then becomes distinctly worse around days 3 to 5 instead of steadily settling down. The pain is often deep, throbbing, or sharp, and it can spread along the jaw or up toward the ear on the same side.
Dry socket pain often feels out of proportion to what the area looks like from the outside. The socket may look empty or have lost the dark clot, but appearance alone is not enough to diagnose it because healing sockets can also look like holes.
Pain medicine may help only a little, and the pain often keeps pulling your attention back even when you are resting. That pattern is the main reason to call the dentist rather than wait it out.
I am the same way! I got all for of my wisdom teeth extracted about 9 days ago and Any little thing I feel I automatically think it’s dry socket. I have been going crazy googling and calling the dentist office asking all the questions. I’m so scared to eat anything. All I’ve been drinking is yogurt. I completely shut down thinking the worst . I’m so tired .. I’m just glad the pain is mostly gone.

Jaw stiffness points more to muscle strain than socket trouble
Jaw muscle strain causes pain when you open wide, chew, yawn, or hold your mouth open for cleaning. The sore spot may feel more like the cheek, hinge of the jaw, or muscles along the side of the face than the extraction hole itself.
This kind of pain can peak a few days in because bruised muscles tighten up after the procedure. Lower wisdom teeth, long appointments, and difficult extractions make this more likely.
The pattern that points to muscle strain is movement pain. If the pain is clearly worse when you try to open, but calmer when your jaw is at rest, that leans away from dry socket.
Dude same. Everyone just says how shitty it is but NO ONE tells you about the mental anguish of constantly being paranoid about eating right, swallowing right, etc. I'm currently on day 5 and it hurts so much to open my jaw wide enough to fit 2 fingers in there! I've even been doing jaw stretches and stuff. UGH, wisdom teeth removal is so icky

Food packing and nearby teeth can mimic a setback
Food caught in the socket can cause pressure, tenderness, bad taste, and a sudden sense that healing has reversed. The pain is usually more annoying than severe, and it often eases after gentle cleaning exactly the way your dentist instructed.
The teeth next to the extraction can also feel bruised or sensitive for a while. Bone, ligaments, and gum tissue around those teeth were stressed during the removal, so the pain may seem to come from the wrong spot.
A nearby tooth ache or a brief sharp twinge does not automatically mean infection or dry socket. If your dentist has checked the area and the pain stays mild to moderate, time is often part of the answer.
Hi OP! Just wanted to say thanks for this post - you put my mind at ease. I have developed a similar sharp pain on the second tooth from my wisdom tooth extraction site. Post op day 7 and I was worried! ESP since I went in today for my review and my dentist said it’s recovering very well and I have no infection. I guess I just have to wait for it to pass…!

The wisdom tooth recovery timeline is uneven but should trend better
The first two days are usually the most swollen. Days 3 to 5 are when people often start testing food, talking more, and moving the jaw more, so pain can briefly feel more noticeable even as healing continues.
By the end of the first week, many people are improving but not fully comfortable. The hole can stay visible much longer than the pain lasts, especially after lower wisdom teeth or surgical extractions.
A timeline that still fits normal healing is one where each few days are better overall, even if one afternoon is rough. A timeline that needs a call is one where day 5 is clearly worse than day 3 and keeps worsening.
Same, I had my wisdom tooth and the molar right in front of it pulled. I think that’s why it’s taking so long to heal. I’m on day 9 and the pain is completely gone. Still have a huge hole but not like a dry socket. There’s no pain. It’s just healing slowly but it is healing so just hang in there it’ll get better. ❤️🩹

Common questions
Is pain 5 days after extraction normal?
It can be. Mild to moderate soreness, tenderness with chewing, and jaw stiffness can still be normal on day 5 if the overall trend is slowly improving.
How do I know if this is normal healing or dry socket?
Normal healing usually improves overall, even with short flare-ups. Dry socket usually gets more intense around days 3 to 5, often throbs or radiates, and may not respond well to usual pain medicine.
Can dry socket start on day 5?
Yes, day 5 is still within the usual window when dry socket pain can show up. The key clue is worsening pain after earlier improvement, not just a sore hole.
Why is the pain throbbing after wisdom teeth removal?
Throbbing can happen from normal inflammation, but strong throbbing that spreads toward the ear or jaw and keeps intensifying is more concerning for dry socket. Throbbing that mainly shows up when chewing or opening wide can be muscle-related instead.
What is a normal wisdom tooth recovery timeline?
Most people have the most swelling and soreness in the first few days, then improve through the first week. The socket can stay open for weeks, but the day-to-day pain should gradually get easier, not harder.
Match the pain to the pattern before you assume the worst. A sore jaw with pain on movement, a tender socket that is gradually settling, and severe radiating pain on day 5 are not the same problem.
If your day 5 pain is severe, getting worse, or spreading toward the ear or temple, call the dentist who removed the tooth. If the pain is mild to moderate and the overall trend is slowly better, keep following the aftercare instructions you were given and give it a bit more time.
